Service requests are required to be submitted on Judicial Form SER-001.

If an eviction or other levy under a Writ of Possession or Execution is to be enforced, Judicial From SER-001A must also be included.

Wage Garnishment requests must be submitted on Judicial Form WG-001 and include an Electronic Writ Declaration (if the Writ was filed electronically).  The Wage Garnishment Application may also require the use of Judicial Form WG-035 (Confidential Statement of Judgment Debtor's SSN).

Any levy request (wage garnishment, bank levy, etc.) where the judgment is based on personal debt must include a declaration regarding address verification.  CCP 683.110(d)(3) defines "personal debt," and CCP 684.130 describes the declaration.  If the judgment is not based on personal debt, the Sheriff's Office requires a statement that the judgment is not based on personal debt (the statement may be included on the SER-001 instruction form).

The Sheriff's Office does not provide copies of judicial forms.
